Key: Kurt Warner threw a five-yard touchdown pass to Anquan Boldin with 1:54 left and Arizona clinched the NFC West for the second consecutive season, its first back-to-back titles since the 1974-75 seasons. The Cardinals had blown a 17-0 lead.
